Please fill out the form below to book an appointment for a minor ailment with one of our qualified pharmacists. All information must be entered as it appears on each person’s provincial health card (if applicable).

• Eligibility for assessment and prescribing for minor ailments will be assessed by the pharmacist at the time of your appointment.

• If you don't have a valid Alberta Personal Health Card, professional fees apply.

• In the event a prescription is written for this service, the cost of the medication is not included in the assessment. The cost will be covered by you or your private drug insurance.
